Carpal tunnel syndrome causes numbness, tingling, and weakness in the hand when the median nerve is compressed as it passes through the wrist. Many people in mild to moderate stages want to try non-surgical care first. Here is how conservative treatment works and when it makes sense.

Common carpal tunnel symptoms

  • Numbness or tingling in the thumb, index, middle, and part of the ring finger.

  • Symptoms that wake you at night or flare while driving, typing, or holding a phone.

  • Dropping objects or weakness when pinching or gripping.

  • Aching that can travel into the forearm.

Why the neck and forearm matter

The median nerve runs from the neck down through the shoulder, elbow, and forearm before reaching the wrist. Tight forearm muscles, trigger points, and irritation higher up the arm or in the neck can add to symptoms that feel like they are only in the hand. That is why a full evaluation looks at the whole arm, not just the wrist.

Conservative treatment options

  • Soft tissue work such as myofascial release and trigger point therapy on the forearm and hand.

  • Therapeutic ultrasound to reduce local inflammation and improve tissue flexibility.

  • Nerve gliding and stretching exercises you can do at home.

  • Adjustments to the wrist, elbow, or neck when restricted motion is found.

  • Ergonomic changes and night splinting to keep the wrist in a neutral position.

These therapies are described in more detail on our chiropractic therapies page. For stubborn tendon and soft tissue pain in the wrist and elbow, StemWave therapy may also be part of the plan.

When to see a medical doctor

Constant numbness, visible shrinking of the muscles at the base of the thumb, or worsening weakness can signal significant nerve compression. In those cases you should see a medical doctor for nerve testing and to discuss whether injections or surgery are needed. We refer patients when their exam points that way.
